Template looking weird

Hello, I chose a free template, but it is looking a little off and I can't fix some of the fields... For example, the NAME field, is adding first and last name, but the light grey boxes that are in it where they type, is overlapping the text.  The Phone Number field is adding two boxes to be filled out, which I don't want..And I don't want it to say "Company" and "Email" under the box that they fill out, because it is already on the side...

The theme applied to your form is quite detailed and caused certain fields to show differently only while editing the form.  If you view the form in your browser, it should show correctly:
