    Do e-mail notifications stop working when you alter the source code?

    I made a form with email-notifications, and it worked perfectly. However, I needed to change the source code of the form for some extra functionality.

    I altered the source code, uploaded the html file on Dropbox, and the form does exactly what I want it to do, except for the email notifications. I don't receive any notification anymore. 

    (The email notifications use conditions)

    Have you checked in your html file if the javascript code at the beggining is still there? it should go between tags <script>  </script> )  It should look something like this:

    The javascript is still there, but I did make modifications. I added function fillEmailAddress() wich is called when a certain dropdown calls it's onchange method. The rest of the javascript code is unaltered.

    The method itself does what it should do, so I don't think there is an error in the code.

    Could you please tell me if the form is already embedded into a page, may I have the URL in order to check the entire code?

    It is embeded on a google sites page, but this is only accesible to people from our company. I did however copy the original source code to a html file again, and added the exact same javascript code. Now it does work..

    Must have messed up something before :/
