Estou tentando resolver um problema no formulário que segue a URL abaixo. PORQUE NO E-MAIL QUANDO ENVIO NÃO CHEGAM O LOGOTIPO E O CABEÇALHO DO FORMULÁRIO?
Já fiz várias simulações e não estão chegando no e-mail nem o cabeçalho nem o logotipo do formulário. Podem me ajudar?

As I can understand form header and logo are not included in the email that you receive upon submissions.
Here is a screenshot of the form header and logo in your web form:

I would suggest you to add the logo and form header to the email template, it should appear in the emails once you do the change.
To edit the email please click on the email icon and click on the edit button, if you want to re-do the email, simply delete it and add it once again to restore the default changes:
