How send a form for some one to sign and return back to me?

I need help understanding how to send a document to someone for them to sign so I get a signature.  It is an agreement between us.

Do you mean to say that you want to create a workflow based form where form can be submitted by user and then it is sent to some other user or admin to approve it by adding signature?
