How to have appoointment slots with multiple locations

Hi there! I discovered the appintment widget which is sort-of what I'm looking for but not quite. Wanted to see if someone could help me out with my question. I'm trying to gather information for someone to book a location (out of two possibilities) and a time slot (with only one available out of 4). The appointment works on the scale that I can just input the date/location/time slot but that is a lot of words for a choice, I was hoping to make it more intuitive. Is this possible?

I think it can be achieved better using the Unique Question using multiple fields feature instead.

The only downside to this method is that, it will not be immediately known to the user that the time/location slot is no longer available. The option will always be available for selection -- and the user will only know it is no longer available when they start to select it.

I'll create a demo form also to better show you how I meant. I'll get back shortly.

You can only select the 2 dates available. The location/time is also limited only to those you specify.

The unique slot is a combination of the date + location/time field. 

So user cannot select slot on the same date + same location/time.
