I have two forms embedded into my school's website.  All has worked perfectly for a few weeks and received over 700 submissions, until this morning when my Sophos protection detected 'High Risk Website Blocked' with the following explanation:

    High Risk Website Blocked
    Location: form.jotformeu.com/form/20854098205353
Access has been blocked as the threat Mal/HTMLGen-A has been found on this website.
Return to the page you were previously viewing.
    sophos endpoint security and control
and according to Sophos it is this - http://www.sophos.com/en-us/threat-center/threat-analyses/viruses-and-spyware/Mal~HTMLGen-A/detailed-analysis.aspx
Any ideas what has happened and has anyone else had this issue?

I have forwarded the case to our Developers. We will get back to as soon as we got updates.

Can you contact them and ask them to whitelist form.jotformeu.com? Since you are a customer they are probably more likely to listen to you.

If they contact us and let us know the phishing form we will disable it immediately.

I contacted Sophos and it turns out that this was a false positive and this has now been re-categorized and all is now working fine.

    Thanks for your help.
