Need assistance creating custom form with condition logic

Hi! I want to pay for someone to make the forms for my firm. I wan to conduct an assestment of a client´s company. I need the client to answer questions and depending on the answers new questions come up. At the end, depending on the answers, the suggested services are detailed along with the Price.
