    Can you help to reactivate my suspended account?

    Dear Jotform,

    Kindly help to reactivate my account "perwiratama" which is now being suspended due to terms use violation.

    I will remove the guilty field right after it is reactivated. Thanks

    I've checked your account,Your account is suspended due to this form. Because you had asked a password from users and this situation is not valid for our terms of use. And I deleted the forbidden area (that you can see at the following screenshot) from your form.

    We could reactivate your account. But first, you have to prove us that you are working for 'perwiratama' and not stealing shipping passwords.

    You can send us your account information or If you want to send us a document as a proof, you can send an e-mail to 'jotformsupport@gmail.com' and please indicate subject as " regarding thread #950355'
