How can I remove the slider from my form? I would like it to show seemless as 1 page

Like this form- http://steelcitypt.com/schedule-online/ The page already has a slider, but my form is showing a slider in the frame as well

If I understand you correctly, you're referring to the scroll bar of the form where it is embedded on a webpage. Did you embed the form using the normal embed code?
