    I am receiving submissions with their name only. Where is the rest of the information?

    We apologise for the inconveniences this issue has caused you. I am not able to reproduce this issue from my end, i have just made submission of this form and all looks well, all fields filled are submitted to your account, i am using IE8.

    This would be a browser related issue. If your user's browser's javascripts are disabled, the user might be able to by-pass the required field checks. So, it is possible but it should only happen in very few cases. Kindly let them enable browser's javascripts, enter the values and confirm if you are able to see them when submitted.

    Try using another different browser too. Lets know if this worked out!

    Thank you for your response.  I too was unable to reproduce it.  The
    strange thing is that I receive a single email with 7 names but no other
    information.  They are consolidated into one email even though they have different times
    on each record all within 24 hours.

    Hello,

    Would it be ok if you add at least a Captcha field at the end of the form? I see that the direct link to your form was in your website. This could probably be caused also by bots/spam. 

    You may check the same idea on this thread How-to-Prevent-Spam
