Try using another different browser too. Lets know if this worked out!

Thank you for your response. I too was unable to reproduce it. The
strange thing is that I receive a single email with 7 names but no other
information. They are consolidated into one email even though they have different times
on each record all within 24 hours.

Would it be ok if you add at least a Captcha field at the end of the form? I see that the direct link to your form was in your website. This could probably be caused also by bots/spam.
