Settings on a Survey - How can I modify?

I have a form https://form.jotform.com/62665735561160 that has 2 issues that I'm not sure how to resolve. This is my first form!

1. If a responder selects one of the radio buttons and then changes their mind before submitting, they've indicated they can't change their selection.

2. One the last question, we would like the ability for users to select more than one option, but currently they can only select just one.

If I make changes and republish the survey, does it generate a new link?

I checked and test your form https://www.jotform.com/62665735561160 and I understand what your question was about.

For #1

You must be referring to the question "Setup Help - Gym..." .

Once selected you can no longer unselect/deselect the option.

In Radio option button field, if the available option is only one.. you cannot deselect it anymore once selected. Since you are using the Radio option button field, it has this characteristic by default and cannot be changed. 

My suggestion is to use the Checkbox field instead.

For #2

You cannot select more than one option in Radio option button field. This is the nature of this field. 

If you want the user to be able to select more than one option in selections, use the Checkbox field instead.

You can go ahead and update your form https://www.jotform.com/62665735561160 in the Form Builder and it will still retain the original URL link.
