    Multiple entries on a form

    I want to be able to add multiple items in a single form.

    1) I enter in all my personal details to apply for a dog licence

    2) I then have 4 dogs to licence and instead of having to enter in personal detail for 4 dogs each time I want to add another dog against my details and also it will calculate the cost of the number licences applied for.

    You could accomplish Part Nº 2 ,  by adding conditional logics

    Another way is by adding form collapse in between, to hide sections for additional Dogs .

    Whichever method you choose , I think you would have to build a LONG form  , and add all necesary fields for additional dogs.

    Here I leave you my attempt, feel free to clone it 

    I have used conditionals on this example and you can customize it to your needs


    Regarding to calculation on the payment section , you can add a quantity option on each product, below you can see my example


    And then you can customize it
