I can't reset my password.

    I provided my email and it said an email with password reset instructions were sent. I still haven't received anything.

    May I ask what is your account username? I checked your email address brkhoh@*****.com but there doesn't seem to be an account connected to that address. Only guest accounts were connected to your email address. Please note that guest accounts are created when a user creates a form without logging in to their registered account. It is possible that you are not getting any password reset instruction because there wasn't an account created yet.

    The guest account named guest_62416492661054 had this form: http://www.jotform.us/form/62417099154156. Is this one of your forms? If yes, then this means you do not have an account yet. Please signup for a Free account and make sure to use the same email address so that the form that was created on the guest account will be transferred to the Free account.
