    Why was my IP banned?

    Recently, I discovered that none of my JotForms were working... I found out that my IP: 24.131.129.234, was banned. It was unexpected and I still have no idea why it was banned...

    Your account was suspended due to phishing the some of the forms were asking for game account information. Please note JotForm can not be used in any malicious internet activities this is against the terms and conditions of use our Phishing filter will automatically suspend any account found to have been involve in this.

    I am certainly not involved with phishing... I have a game server for Minecraft that runs and accepts donations from people who wish to contribute. Since I must pay a monthly fee of $67 to host the server, it is completely necessary to accept donations. I don't ask for passwords or anything, just the username in order to give them benefits in the game. If it's necessary, you can take that form down since I did not realize this was against the terms and conditions.
