I have 2 forms I am currently using. A MICHIGAN order form and a OHIO SOUTH order form. When the Ohio South order form comes thruough, I get only one copy, which is correct. When a Michigan order form is submitted, I get a copy of the Michigan order form and the the same information is copied on a Ohio South order form and sent again. So essintially I am getting 2 order forms with one cusotmer's information on it. One sent from Michigan and one sent from Ohio South. When I created the Michigan order form, I created the Ohio South first and used it as a copy to create the Michigan form. So I know that may have something to do with it, I just don't know how to fix it. Any information would be great. Thanks.

The issue lies with your Michigan form as it contains 2 notification setups. See image below. Removing what should not be there would solve the issue.
Removing OHIO SOUTH Notification should do it.
To remove it go to Setup & Embed > Email Alerts > OHIO SOUTH > Delete Email
