Problem with (Square) credit card fields

The final four fields in the square credit card section of my order form are

not working, leaving my customers unable to place orders. Please advise.

Apologies for any inconvenience. However, I'm not able to find the form on the website you added in your post.

I have checked the two recent forms you're working on but I see that only one of the forms has Square integration in it and I'm able to see the last four fields of the credit card section.

May we know which form are you working on, please?
