I am having several people contact me saying the file upload function isn't working for both of my surveys. But, it is working for other people and for myself when I try to run test entries. How can I diagnose this problem?

We're sorry for the inconvenience you had. Our developers did some changes recently on upload field and that might cause this problem, but it is strange it is happening randomly. Anyhow, I did some necessary changes on your account, please test and let us know if you're still experiencing this problem.
Unfortunately, there is no way to observe failed uploads.
What you could do to improve user experience while uploading their files is to enable Multi-file upload:
That way your users will be able to see the progress of uploaded files:
