My form cannot be displayed because of the error "Form is Disabled." However it is listed as "Enabled"

    I have checked your form and found out that it is in auto-suspension status. One of the forms in your account (that is in the Trash Folder) has an image of "Credit Card" logos in it. Our anti-phishing system detected that form and suspected it for phishing activities.

    I have deleted the image of credit card logos in the said form and reactivated your account.
