The system's filter scans on every form and checks the fields for possible usage for phishing. If it finds anything, it automatically suspends the account. This is similar on how anti-spam filters work.
Upon checking your form, I did not find anything malicious in it so I went ahead and reactivated your account. You should be able to access your account again.
