Hello! We can't see feild vailues in submissions besides ID, TIME and IP fields.
Fields empty in Excel and CSV extracts too.
Can you help us?

If I understand you correctly, you're having a blank data in your submissions except ID, TIME, and IP fields.
I was not able to replicate the issue. I cloned your form and I was able to receive all the data. Here's a screenshot:
This is most likely happening because the "Europe Datacenter" option is not enabled on your account. Here is how to enable it:
Go to your https://eu.jotform.com/myaccount/data page. Enable the option here:
I noticed that you're a sub-user, you need to do this also to the main account. Please check this and see if the issue still persists.
