I receive multiple emails when someone submits a form

I am not sure the reason, but several people when they complete the form the system generates several emails notifying me that they completed it. In addition, the form is duplicated in the submissions section...it has been duplicated up to 5 times at one point.  Please help! 

I've viewed your form submissions and can confirm that you are getting duplicate submissions. I'm not sure what's causing, but I would suggest adding a captcha in your form to prevent this from happening again.

I would recommend using the Google reCaptcha widget so users won't need to type if ever they'll be detected as a possible spam and needs to verify.
