Why my account is disabled?

    i PUBLISHED FORM BUT IT IS SHOWING FORM NOT FOUND

    I checked your account and it was autosuspended by our anti-phishing system that detects suspicious forms and disables them.

    I checked the mentioned forms and problem is in username and password fields that you placed which are against our Terms of use.

    I removed those fields from your form and please avoid putting them back or our system will disable your account again.

    I also activated your account so you will be able to use it.
