I am trying to set up conditions to my form but they are not working

I am trying to set up conditions to my form but they are not working. I would like to set up show/hide page conditions and it let me add them but when I go to the preview it isn't skipping to the next page like I set it to. 

Please help.

Well, your conditions are working as far as I could see it. You have to click the next button to move to the other pages based on your condition. But actually, why redirect if they are in sequence? The question in your condition #4 to skip to page 2 is on page 1 so whether Yes or NO is selected, the form will still move to page 2. The condition is not needed as far as I understand your current flow in the form.
