I am not sue why its suspended.

Where you're asking for Credit Card information, here is a screenshot also of the guilty fields:
Please do note that asking for Credit Card details goes against our terms and this is why your account has been suspended, this is also stated in our terms page so I would suggest you to take a look on that so you avoid keep asking for not allowed info in your forms: https://www.jotform.com/help/8-Terms-of-Use
In order to re-activate your account I had to remove those fields from your form, also it is a must to use one of our payment integrations if you want to process payments in your form, here's the link to our current integrations: https://apps.jotform.com/category/payment_processing
But do note that the Credit Card info will not be stored anywhere in the JotForm submission, the payment integration will help you to process the payment only, you could also use on of the payment gateways that will help you to get the payment authorization only so you do not charge your submitter immediately: https://www.jotform.com/help/292-How-to-Enable-Payment-Authorization

Seems like you asked on this form for login credentials; however, I can see you have already removed the fields from your form, I have re-activated your account now and you should be able to load your forms, please check and let us know.

Have my forms been disabled for the same reason? I have one asking for address info to collect orders and another getting payment info & authorization, but I used a template provided on your website for it, so I don't see why it would be disabled...
