I have checked the email: firstname.lastname@example.org and the account is active on my end, therefor it is not blocked. Please try to clear your browsers cache and try to login again. If you forgot your password you can do the following steps:
Go to www.jotform.com and on the bottom right of the page you will see an area to reset your password:

Step 4: Check your email
