We had this problem a few weeks ago. We send the following link to our schools to complete the details and we are not receiving them again. There was a block on the form, but that was fixed by someone at your company, but it seems to be doing that again.

I have tried to complete the form myself and submitted it, but haven't received the form.

I checked your account. Your e-mail was in the bounce list.
I removed it from the bounce list. Now it should work fine.
