my forms have been disabled

Hello, I am trying to make forms for my webclass and my teacher wanted us to put a password field, i saw that it might be an issue so i changed it to the word key, but when i try to preview them they are still disabled and now i cannot finish my assignment. My teacher is insistent that i use this website instead of just doing the coding myself i really need my forms to be enabled. I have added a screenshot of my teachers instructions of how to upload this

i need my page to be enabled please so that i might follow them and add them to my assignement

Your form was automatically suspended by the sytem's phishing scanner. Upon checking your form, it appears that there is a field that appears to be suspicious. Can you please provide more details what the field "Key" is about? It seems that this caused your account to be suspended by the system.

someone said putting key instead would work it is a fake Password button. My teacher is a huge idiot and she doesn't reply to e-mails in a timely manner. She wanted a password button on the form, but you would think since she is forcing us to use jotform that she would know that it's not allowed, i am just trying to get a decent grade in this class, you have no idea how incompetent this teacher is. she uses outdated youtube videos for programs, and then doesn't put all the information or tutorials you actually need that screenshot is a screenshot of part of the assignment directions, please i swear i am just trying to do homework, you have no idea how exasperated i am with this teacher. which btw she's a college teacher.

Anyhow, upon checking again your form, and it shows to be active now. Feel free to modify your forms to your desire, just keep away from including fields that may appear to be suspicious. Otherwise our system will again suspend your account.
