When I use the embed source code option for a jotform and paste it in mailchimp template builder the radio buttons and submit button dissappears and all thats left are the text.
What could be the cause of this and how can I resolve this?

It could be MailChimp restrictions which do not take the code. Try embedding the form with the Iframe code: https://www.jotform.com/help/117-How-to-Inject-Custom-CSS-Codes

Before you embed the code, paste it in a text editor like WordPad, you will notice two part of codes, only copy and embed the Iframe part in your webpage, example:
