Hi, my form was working for a couple of months and just stopped recently. It shows on my computer but no one elses...I had to sign up again and now I think the form is online but it's saying my email address is blocked by jotform.
Can you let me know whats going on so I can continue receiving emails?

We are sorry that you having this issue. I have cloned your form and was able to submit this form successfully and recieved notification both on my email notification account and jotform account.
You can also try to change your Sender Name to JotForm, and your Sender-Email to email@example.com and then do a test?
You can also request your ISP or mail provider to whitelist jotmail's IPs shown in the link below;
