I checked your form on an Android device and it looks like I am able to upload an image successfully.
I also tested a cloned version of your form and I was able to upload and submit the image without any issues.
Do you still encounter the issue on your end? Which device and browser were you using when you encountered the issue? Please try clearing your browser cache and your form cache then try uploading again.
