I need someone to help format a form for me?

I'm creating a player profile for my team. I have put all the information in the form, but it just doesn't have a clean appearance. Also, I need each user uploaded photo to be the same size so each page will look the same.

Also number field next to Last name field, I didn't get what is it for?

And regarding Image field, I added sub-label, but probably you could change that. If you need additional assistance with that field let us know how would you like to have that field behave.

The number beside the name is the Player's Jersey Number. I would actually like the Player name and jersey number to be the title or heading. 

With the image, I just need each photo submitted to have the same dimensions so each page will look the same when printed. 

I am not sure if I understand your question correctly. Do you mean to have the field names to be displayed in the center or display them as heading in the Thank you page/email notification?

Unfortunately, it is not possible to restrict the file dimensions using file upload. However, you may mention the file dimensions in the sub-label and can restrict the image by its file size.

Is it possible to have all this one one page when printed?
