Hi there, the form created is available for view in the preview section.
However, unable to view the actual form, only displaying the below screenshot

Sorry for the inconvenience that may have caused you today. Upon checking your form in your facebook page I found out your form is working fine. Can you please delete your form cache and cookies then let us know if the said issue still persist for further assistance and investigate.

Hi there, I'm still viewing the same screen, as per my 1st message to you.
Is administrator of the page only can view the 'setting' screen, instead of the actual form?
I did asked others to view the form, which is our fans, but no display, only a blank page.

Hi, I am not an 'administrator' and using public internet, I can view the form just fine (similar to previous stated by my colleague pinoytech).
I can see the page like this.
Perhaps a cache on your browser is causing this anomally on your viewing the facebook page.
Could you try clearing your browser cache and view the page again.
