I keep getting the form not found error

it is letting me create a form but then this comes up when i go to preview

You seem to be asking a credit card details in the form. Please note that we don't allow this type of form. It is against our Terms of Use (http://www.jotform.com/terms/). I have reactivated your account, but I have deleted the said fields. Please refrain from adding them again. We cannot unsuspend you again the next time.

If you'd like to collect a payment at a later date, you can use our Payment Integrations and enable authorization only.
