I've found a problem with the attachments uploaded via Google Chrome on Mac. Apparently the problem started on November 8, 2016.
When uploading a file via Chrome, the frontend form says that the upload was successful, but in the backend I do not see any trace of any attachment.
Uploading the same file via Safari or Firefox (always on Mac) works correctly.
The latest stable update for Google Chrome seems to be from Nov 9.
We are way below the upload limits (1.5Gb used of 10Gb available).
Do you have any idea/solution?
I'm running on a Macbook Pro Late 2011 with macOS Sierra 10.12.1
Google Chrome version 54.0.2840.98 (64-bit) (so it's not the Nov 9 update which ends with .100)
Safari version 10.0.1 (12602.2.14.0.7)
Firefox version 49.0.2

We are sorry for the problem you are having.
I'm escalating this issue to our developers for further inspections.

to add on what I previously wrote: I tried with another form we have, much longer than this and with many more attachments to be uploaded: https://form.jotform.com/62761469314965
I had no problem with this form, and all attachments were properly uploaded and displayed in the submission list. Moreover, this form is connected with box.com, and all attachments were correctly saved in box as well.
So...I have really no clue what's wrong with the other one...

Are you still experiencing this problem? I did a test submission on my end, on google chrome for mac and the file was uploaded successfully.

unfortunately we are still experiencing the same problem on the Frontend side. By modifying the entry in the "Submissions" inside Jotform, I was able to upload the file...
