Restricted dates in calendar pull-down

Thanks for your feedback on earlier queries.  I have placed restrictions on available dates on one form, but fail to arrange this on another:  'George Christmas Set Menu Booking Form'. I want only December 2016 dates to be bookable, excluding Sundays and Xmas Day.  I have made these restrictions, it previews perfectly, but when saved it still shows all dates available on my website.
