If they answer no we hit them with a short answer question asking for advice on improvements of our service; however if they answer yes, is there an easy way to set up a referral of our business right from within the survey?

3) Here is an idea:
You can add another email fields, where you could show it if the answer "Yes": https://www.jotform.com/help/316-How-to-Show-or-Hide-Fields-Base-on-User-s-Answer
Then, you could create and customize and autoresponder to be sent to the email address inputted on that second email field: https://www.jotform.com/help/26-Creating-a-Form-Autoresponder
And finally create a condition to send that customized autoresponder if the "second email field" is filled: https://www.jotform.com/help/167-How-to-Send-Email-Based-on-User-s-Answer
