Form data not transmitted to email. have used yahoo and domain based email accounts, no luck.

I checked your mail logs and everything is being sent successfully. Here's a screenshot:
However, I noticed your conditions is not set properly. Please complete the change email recipient condition and try again filling up your form and manually pressing the submit button. Here's the screenshot:
