Why is only the top part of my form saving?

Only the first 6 fields or so are saving on my form. The rest is not. Have I missed a step?

I checked your form and far more than 6 fields are showing:

Are the fields still not showing on your end?  As far as I can tell, the form is should allow for saving changes and will show those changes.
