I need to upgrade my account i guess as the forms are no longer on my website.

Could you please let us know the form ID or the username under which you have the forms disappeared because this user that you're writing from is a guest one.

I do not know my log in because i was not the one that created it. The company that managed my website created it and they no longer manage it.
this is one form:
all forms on the website are currently not working.

I see that the account is now holding Free subscription plan and the forms are disabled since the account has reached the form views limit. It is required to upgrade your account in order to reactivate the account.
You may request for a password reset from the login screen so that the password reset link will be sent to the account registered email address.
