google drive stopped integrating

Google drive stopped being integrated with our forms on Nov 7, 2016. What should we do to restore it? thank you!!!

I tested our Google Drive integration and it looks to be working at this time.  If yours has become disconnected, removing the integration and then adding it back will reconnect it.
