I have inquired about this problem about a month ago and never received a response. I am still having the same problem. After creating a form, when I click "View Form", I receive a blank page (all white) as if it hasn't loaded.

Thank you for your response. I see that there had been a response; somehow I missed it.
The name of the form is "CVESD PE Reporting Form Teacher November 2016".
I can view the form if I click on "Edit Form", then "Preview" and "Open in a New Tab", but I prefer to view the form using "View Form" after clicking on the "More" dropdown menu.

I login to your account and test if the view form in the More dropdown menu is not working. I was not able to replicate the problem, it is working fine on my end. Please check the screen animation:
