What is the problem with my form?

Hello,

I've created an upload form and made the integration with my google drive. It was working greatly for a while and even I've send some test submission by using the preview tab. However, now there is a problem which I couldn't understand how to fix. It says the form is disabled but the form is enabled, I've checked it many times. Can you please show me a way how to solve the problem?
