Looking for template form?

I'm learning how to use Jot Forms.  Is there a template form similar to https://form.jotform.com/60801300050939 that I could start with?  Are there members of the Jot community that will create forms for a fee?  If so, what is the best place to find the?  Elance?

If you have a very specific custom form that can only be created from scratch, you can also send to us a request for it. You can use the Request Form option button when your logged in to your account in the Form Builder.
