Can you provide to us the link to the site where you have embedded the form so we can further check the issue? If you can provide us a screenshot of the error you encountered so we can further investigate the issue. This is what it looks like for me when I try to use the recorder:

I checked the test site that you have provided and I was able to replicate the issue. The Record and Play button was displayed.
However, when I try to record audio (click Record button), the error appears.
As per further investigation, it seems that issue occurs because the site is non-secured, it is under HTTP.
To properly use the audio recording widget, please embed your form to a secure site which can be accessed through https:// URL.
