1. In Form Builder, click on Add Element on the left side of the page.

2. Now, in the Form Elements menu, under the Widgets tab, search for Zip Code Auto-Suggest and then click on it. Or, just drag and drop it to where you want it to be on your form.

3. Next, in the Widget Settings menu that opens on the right side of the page, set up the Google Maps API Key option to add the key that lets the widget talk to Google Maps so it can pull up real, accurate address suggestions as your users type. . 
4. Once you’re done, click on Update.
