What is Form 1096 used for?
Form 1096, Annual Summary and Transmittal of U.S. Information Returns, is a cover sheet that goes with paper information returns you send to the IRS. It summarizes the number of forms and the total amounts reported on Copy A of Forms 1097, 1098, 1099, 3921, 3922, 5498, and W-2G.
Do I need a separate Form 1096 for each type of form?
Yes. You need one Form 1096 for each type of information return you file on paper. For example, if you send both 1099-NEC and 1099-MISC forms, each group gets its own Form 1096. You don't need Form 1096 if you file your information returns electronically.
When is Form 1096 due?
Form 1096 is due on the same date as the returns it covers. That's early in the year for most 1099 forms (Form 1099-NEC has an earlier deadline than most others) and later in spring for Form 5498. Check the current Form 1096 instructions for the exact dates and the mailing address that applies to your state.
How do I fill out Form 1096?
Enter the filer's name, address, and contact person at the top, then either your employer identification number in box 1 or your Social Security number in box 2 (not both). In box 3, enter the total number of forms you're sending, and in box 4, the federal income tax withheld shown on them.
Box 5 is the total amount reported on the forms, and the instructions tell you which box on each form to add up. Check exactly one box to show the type of form you're transmitting, then sign and date the form.
Can I mail a printed copy of Form 1096 to the IRS?
The IRS scans Form 1096 and Copy A of information returns, and it says the versions printed from its website aren't scannable. It asks filers to use official printed forms for paper filing, and it may charge a penalty for filing forms that can't be scanned. Many people use a filled PDF as a worksheet and record, then copy the figures onto the official form or file electronically. Also note that if you file 10 or more information returns in a year, you generally have to file electronically. Check the current instructions to confirm what applies to you.
