What is Form 8821 used for?
Form 8821, Tax Information Authorization, lets you name a person, firm, or organization who can inspect or receive your confidential tax information for the tax types and periods you list. Accountants, bookkeepers, and payroll companies often ask for it.
It doesn't let the designee represent you before the IRS or act on your behalf. For that, you'd use Form 2848.
What's the difference between Form 8821 and Form 2848?
Form 8821 only gives someone access to your tax information. Form 2848, Power of Attorney and Declaration of Representative, lets an eligible representative, such as a CPA, attorney, or enrolled agent, represent you before the IRS.
If you just want someone to see your account records or copies of your IRS notices, Form 8821 is usually enough.
How do I submit Form 8821 to the IRS?
You can mail or fax Form 8821, or upload it through the IRS's online submission tool. The form instructions list the right fax number and mailing address based on where you live.
Generally, the IRS must receive the form within 120 days of the date you sign it, or it won't be recorded on the Centralized Authorization File (CAF).
Can I cancel a Form 8821 authorization?
Yes. You can revoke it by sending the IRS a copy of the original Form 8821 with "REVOKE" written across the top, signed and dated again. The instructions explain what to send if you don't have a copy.
Also note that filing a new Form 8821 automatically revokes earlier authorizations for the same tax matters, unless you check the retention box and attach copies of the ones you want to keep.
How do I fill out Form 8821?
On line 1, enter your name, address, taxpayer identification number, and daytime phone number. On line 2, add your designee's name, address, CAF number if they have one, and phone number. Line 3 is where you list the type of tax, tax form number, years or periods, and any specific tax matters.
Then check any boxes that apply for special uses or copies of notices, decide whether to keep earlier authorizations, and sign and date the form.
