FAQ
What is Form 4852?
Form 4852 is the IRS substitute for Form W-2, Wage and Tax Statement, and Form 1099-R, which reports distributions from pensions, annuities, retirement plans, IRAs, and similar accounts. It stands in for a W-2 or 1099-R that you never received or that has incorrect information, so you can still report your income and the tax withheld.
When should I use Form 4852?
Use it only after you've tried to get the correct form. Start by asking your employer or payer for the W-2 or 1099-R. If you still don't have it by the end of February, contact the IRS for help.
If the form still doesn't arrive in time to file, you can file your return using Form 4852 instead.
How do I estimate the amounts on Form 4852?
Your final pay stub of the year is usually the best source. It typically shows your year-to-date wages, federal income tax withheld, and Social Security and Medicare amounts. You'll also explain on the form how you figured the numbers and what you did to try to get the missing form.
What if my W-2 arrives after I file with Form 4852?
If the W-2 or 1099-R you eventually get shows different amounts from what you reported, you may need to file an amended return with Form 1040-X.
Also keep in mind that the IRS may take longer to process a return that includes Form 4852 while it checks the information.
How do I fill out Form 4852?
Check whether the form replaces a W-2 or a 1099-R and enter the tax year. Then add your name, Social Security number, and address, followed by your employer's or payer's name, address, and identification number if you know it. Fill in the wage or distribution amounts and the taxes withheld.
Finish by explaining how you figured the amounts and describing the steps you took to get the original form, then sign and date it.
