Hi,
There's a workaround for that using custom quantity.
Here's an example:

What you just need to do is to create a separate product, put $1 as the price and name it donation or anything you like. After that, add a custom quantity in it just like the following.
After that, let us know the form URL so we can provide you the custom CSS codes that should be injected to your form to remove the check box and the product pricing after the donation text.
