How can i ensure that the last box shows up when downloading the form

When downloading the form I am unable to see the last box that suggests the reference may be shared. 

Why does this happen? I have played around with the settings but I still can't see it.

Are you referring to downloading the PDF files of your submissions?

If so, it sounds like you may have customized the PDF Design for your form, and that the custom design was created without those fields. In order to correct the issue, I would recommend that you reset the custom design of your PDF files, which you can do directly from the submissions page of your form:

Simply click on PDF > Customize PDF:

When the PDF Designer loads, click on Reset, but do not click on Save after that:
